Reminder Leaflets - Adult
You should take your reminder leaflet with you everywhere you go. The leaflets include possible side effects of AMGEVITA and you should present it to your healthcare provider every time you have an appointment.

If your child is prescribed AMGEVITA, you can carry this leaflet with you. It includes possible side effects of AMGEVITA, and you should present it to your child's healthcare provider every time they have an appointment.

In the event of an emergency, this card can help to give information about your AMGEVITA treatment to anyone who may be caring for you. This card is important to carry with you, as it also contains your healthcare provider's information so that they can be contacted.

Order Sharps Bin
or Alcohol Wipes
Your SureClick pre-filled pens or pre-filled syringes need to be disposed of safely and in accordance with medication guidelines.
Once your sharps bin is full, talk to your pharmacist about how you can safely dispose of your sharps bin and request a new bin. If you would like to order a sharps bin from Amgen, please fill out the form below.
You can also order a pack of 100 alcohol wipes to use prior to your injection. If you would like to order alcohol wipes, please fill out the order form.
Just make sure to have your AMGEVITA pack handy, as you'll need to insert the last 4 digits of the pack bar code to access the order form.
